<abstract displayLabel="Summary">This paper briefly describes two related projects, of which one is completed and the other ongoing. The first project attempted to identify sources of error from an analysis of operating irregularities, a term which will be used interchangeably with system errors in this paper. The second project was concerned with the development of a revised category structure for those operating irregularities which will be compiled in an interactive database</abstract>
