Based on a survey of nearly 1,200 leaders, it finds structural maturity and larger budgets do not always yield better outcomes; leadership saydo gaps undermine culture and speak-up confidence; and AI is rapidly expanding into training, investigations, monitoring and reporting, amplifying existing governance strengths or weaknesses. The report benchmarks headcount and budgets by company size, maturity and industry regulation; examines speak-up program realities, retaliation fears and manager readiness; and highlights regional differences across the U.S., Germany, France, Japan and the U.K. It concludes with recommendations to shift from building structure to reinforcing behaviors, align board oversight with culture, govern AI with human validation and transparency, and prioritize trust, responsiveness and accountability in speak-up systems.</abstract>
