The survey obtained information on employee characteristics that may affect the risk of respiratory disease. The questionnaire was designed for use with both subject and proxy interviews and for both in person and telephone interviews. It included questions on demography, lifetime residence, occupational and smoking histories, hobbies, and personal and family medical history</abstract>
