Siderosis, the accumalation of ferric oxide particles in the lung, was first described by Zenker over a century ago. Enquiries subsequently focused on the effect of this peneumoconiotic process on lung function, generally concluding that it was bening. More recently however, it has become apparent that iron influences bacterial virulence and has a role in host defence against infection. The evidence accrued is sufficient to trigger a reappraisal of health risks in occupational groups exposed to iron. This paper reviews some of the evidence linking iron with infection, and considers what information exists on risk in the occupational setting
