<abstract displayLabel="Summary">La loi de 2004 prévoyait une protocolisation des traitements concernant les affections de longue durée (ALD) et une remise en cause de l'exonération de ticket modérateur en cas de non-respect de celle-ci. Cette disposition essentielle n'a jamais été réellement mise en oeuvre alors que personne ne conteste plus aujourd'hui le fait que l'hygiene de vie et l'observance d'un suivi et de traitements normés sont les fondements mêmes d'une gestion du risque performante. L' assurance maladie s'est engagée dans une voie moins contraignante, basée sur le volontariat, qu'elle teste actuellement au travers du programme Sophia à destination des diabétiques. Nous proposons de généraliser rapidement ce programme à l'ensemble des malades et des affections concernés puis de le rendre obligatoire une fois cette généralisation réalisée, l'objectif à plus long terme étant de parvenir à une application stricte de la loi de 2004 </abstract>
