This study examines the impact of the transition to grandparenthood on mental health and healthcare utilization in China, using longitudinal data from CHARLS and a difference-in-differences design with fixed effects. The results show that becoming a grandparent worsens mental health and self-rated health, and increases the use of outpatient care and out-of-pocket medical expenditures, especially among women and individuals with lower socioeconomic status. Intensive childcare for grandchildren, reductions in working hours, increased retirement, unhealthy health behaviors, and strained relationships with adult children are identified as key mechanisms. The study concludes that the lack of formal childcare services and economic pressures exacerbate these negative effects, highlighting the need for policies that expand access to childcare services and provide support for older caregivers
