Sumario: You can have the most sophisticated, proactive, well-funded safety program in the world, and somehow, somewhere, someone may still a way to get hurt. The key, as every safety-and-health professional knows, is to head off injuries before they happen. However, when an injury happens, it is also important to be able to detect where things went wrong.
