Sumario: The worker turns off the power but doesn't lock out the machine to keep other workers from starting it. Someone down the line doesn't see the first worker and turns the switch on, exposing his co-worker to serious danger. This is the kind of risk that the Occupational Safety and Health Administration set out to eliminate when it issued its lockout/tagout standard, called Control of Hazardous Energy Sources
